I'm pretty sure he's just a lich, who, since he was allied with the Death Knights, broke free from the Lich King's will and happens to train death knights. I don't think he has any lore behind him. The name is just a common structure for liches.

Copied from Wowwiki.com
"In one of the early Death Knight starting quests Lord Thorval refers to Amal'thazad as an Arch-Lich. This only further adds to the question of why Arthas would let go of a Lich, let alone an Arch-Lich. Kel'Thuzad is the only other known Arch-Lich of the scourge. "
If I had to guess, its a naming pattern given to Lich's?

I think the opposite is true - Alam'Thazad decided to change his name to something more scary upon becoming an Arch-Lich so that he could be just as well-respected as Kel'Thuzad.
Kel'Thuzad set the naming fad, and now all the Arch-Liches are doing it!

Amal'thazad is just what they choose to be the Frost Trainer for Death Knights, guess they figured cause Kel and all the other lichs love the frost spells so much, they would use that as well.

Kel'Thuzad set the naming fad, and now all the Arch-Liches are doing it!Because Joe the Lich wasn't scary enough...
