The basic NEED/GREED works pretty good. Just make sure that NEED is only for what spec you are playing (unless previously discussed).
Also, 1 Need win per run, and 1 piece of loot per boss. This prevents one person from getting tons of gear if they are lucky with rolls. It helps with distribution.
It's simple, and easy to follow.
unless you can trust everyone in your guild and or pugs, id go with master looter, bro.

we go win 1 pass 1... no limit on tiers or anything like that. Basically if you win one, anyone of your role has priority over you on rolls until everyone of that role has won...so...if our tank gets a shield off grob and thaddius drops repelling charge, he can't roll on that unless the other tank doesn't want it. For tier we include everyone on the tier token.

An important thing to mention, since dual spec is so common these days: Your mainspec would be what you did FOR THAT FIGHT. For example, if you dps'd all of the naxx 10 run, but for KT, you healed it because they needed a 3rd healer, your mainspec would be healer because you healed that particular fight, not dps.
Sucks for the Offtank on single tank fights...

My guild for Naxx10 and Uld10 just does a need/greed system. But priority is given to those who haven't won anything yet.
So say there are 2 plate dps, one gets a new 2h, next boss a ring drops, even if the person who won the 2h wins the roll it will go to the person who hasn't won anything yet.
Same for Naxx25 guild runs as well, works ok for us, we're all easy going, and like to see guildies get loot.

DKP based system. The only way to go.
There's nothing like being a guildy with perfect attendance and lose a roll to the guy who logs in once a month.
Reward your guildies for attendance. They will love you for it, log on more in turn, and you will progress faster.
WebDKP.com provides a nice addon, and your guildies can check their DKP at any time on the site.

We thought about a DKP system, and decided against it because of hording issues.
As far as I can see, your loot system is overly complicated, and has far too many issues.
And "hoarding issues" aren't an issue. The only way to get DKP is to attend, the only way to lose it is to spend it, hence those who have a lot of DKP deserve to "hoard" it because they earned it.
That's like saying the rich man isn't entitled to the millions he worked for.
They earned their DKP. If they earned it that means they didn't spend it on gear. Which in turn means that other guildies got ups, while they passed on loot. If a major up drops, and they really need it, they should be allowed to outbid the others and grab their up.
A DKP system is like the waves of the tide... the DKP fluctuates and changes so that no one person can "hoard" it all and win all loot at all times.
