This doesn't seem to be much in current heroics, even not often used in raids, except for one or two boss fights in Naxx and Ulduar. As for marking a player, that could be a boss mod doing that.

The only marking standard that almost everyone uses is that if a mob is marked {skull}, it is your first target.
You can also safely assume that a mob marked {cross} is second to die. The last mob to die is usually a mob that is CC'd somehow, and that would be marked with something else.
Other than that, marking standards vary between groups. Raid leaders should tell you why people are marked, so don't worry.

Typically myself and the people I run with used this convention back when marking was needed:
Skull = first
X = second
Star = third If necessary of course
Triangle = fourth Ditto as above
Moon = sheep
Square = Hunter Trap
Diamond = Sap
As others have said... it's not really used anymore, but still interesting. Now we just mark noobs in our group with the Circle/shield/nipple/condom for doing dumb things. lol

Apart from Skull first, X second, it really depends on the people you're playing with. I've run with groups that go Skull, X, Square. Some go Skull, X, Moon. Etc. Others use Star to mark wandering patrols. Some people use the Moon and Diamond for targets to be stunned or sheep'd, some people don't.
My belief is that if you're the person marking mobs and you're running with some strangers in the group then you should tell them the order to attack in.

There are no universal conventions, so tell people why you mark or ask others what their marks mean.
I generally don't use marks in 5-man instances, however we do use them often in raids where you several times have pulls of 7-8 mobs where it's quite important to be burning them down fast. Also usefull on boss fights for group positionning or whatever other important signal you want to give depending on the encounter.

Everyone I have ever run with marks first to die skull, second target X.
The other marks are explained as needed, if they're used.
Sometimes the tank will be the circle, especially in a fight where there is a certain phase when everyone has to stack up.
Moon is usually hibernate or sheep, but I haven't seen either used in a group in so long.

I have mark bound to keys
Shift +W = Skull
Shift + A = X
Shift + D = Square
I can mark targets on the run, and often do so.
Generally, I try to run with the same people so we all use the same marking conventions where the kill order is Skull, X, Square.
In raids, we generally use Star and Nipple to mark tanks - any changes to the normal conventions, we talk about before hand.

I've got all the marks bound to numbers and I leave numlock on for tanking. I changed auto-run to * if I remember correctly. It's how I found out you can get an error message which reads something like 'you have performed too many raid leader actions.'
No one's mentioned it but when I have to mark for repentance (which is never, because who still uses CC and show me a ret paladin who realizes they're a crowd control class. I can say that because I was ret in TBC, of course) I mark it with triangle.
