I still have three points in focused aim and I am not uber geared at all. Bringing the hit cap down is really nice, as I didn't have to start gemming or enchanting for hit until last night. As far as your gear goes, there's nothing wrong using PvP gear for PvE if there's nothing else better that is available to you. I used Titanforged Sabatons until last night for the boatload of hit.

is a good choice if you're hurting on hit. Putting points into focused would reduce your hit cap significantly. There's three gem slots (Including a dragon's eye) that could be redone with Agility/Crit/AP gems. Again, it's a matter of personal preference.
I stacked more hit than I needed, that way I wasn't scrambling when I was looking to replace something. Since stacking 200 hit is ridiculously easy in WotLK, it wasn't too tall an order to pull off. Over time, I was replacing pieces, and either losing or gaining small portions of hit, but always staying above the cap. Last night, I replaced

, which is more suited for my PvE role. Losing the 44 hit put me under the cap, but a 8 agi/8 hit gem and icewalker on the boots put me right on the cap with 3/3 focused aim.
Pulling points out of one talent to lower the hit cap with another is all a matter of preference. To me, pulling some points out of improved tracking and putting them into focused aim is worth it to me, not to mention seizing the gems slots filled with hit and replace them with stat gems. But some people might not think so.

Yes, to answer your question, swapping out my swords after 80 was tough, because I had two of the Wyrmrest Accord swords with titanium weapon chains. Having 122 hit pretty much all in one place was hard to adjust around.

I do not gem for hit at all. I am usually running with my wife (Draenei Hunter), but I do not assume that I will always have her Heroic presence. With 3/3 Focused Aim, you only need 164 Hit rating as a Non-Draenei (131 if you are a Draenei) to be capped.
I did my first +hit rating chant on my boots last week, so it is quite doable to simply get the hit cap from the the itemization of the gear only. I had boots, chest, belt and rings with hit rating on them and it put me almost 30 over the cap.
Taking points out of focused aim means you'll have to gem/chant for hit respectively.
