What actually started this fad? I know it is brought up a lot with kael being in TK and MT, but I can't for the life of me figure out why it has become so common-place for a recurring boss to have " was only a setback!" at every mention of his name.
Explain to the ignorant one please :D

Yes I see it very commonly with the fact that Anub'arak is in the new trial of the crusader instance.

Kael'Thas initially said the line "Don't look so smug, I know what you're thinking. But Tempest Keep was merely a setback," in Magister's Terrace.
Kael'Thas was the first boss to come back from the dead to pester players in two separate instances, and this seems to be a continuing trend with such things as Naxxramas coming over to Northrend as if a legion of adventurers
didn't
completely rape it years ago, and now Anub'arak.
Some players resented the fact that they had to kill Kael'Thas twice, others just found that line ludacrous especially if they've never killed Kael before. Either way, the meme stuck.

Finally an explanation that makes sense.

Correct me if I'm wrong, but Kael'thas didn't really die in TK... right?
Yeah, you fought a hologram or something, that was oddly, stronger than the real deal :P

Crusader's Coliseum was only a setback.
I prefer old Kel'thuzad's saying though.
"Your curiosity will be the death of you."
