I got the
Chaos Theory
achievement when we weren't even trying to do it.
Fighting anomalus in a pug, oh about 3 or 4 months ago. The group is about to wipe. The last person alive is our Shaman healer. The shaman is killed with the boss only having 900 HP left. Before he can reset, the shaman pops his ankh and earth shocks him dead. The encounter had reset, but his HP had not yet reset, thereby giving us the achievement, even though we had been killing rifts.

If you mean to Blizzard, yes, I did at the time. I just thought it was a great comeback story.

I was just joking. I've heard of guilds using bugs to get achievements being 'reported and banned'. I always wondered who would try to get a whole guild banned for something like that.

OP, Thaddius does not wipe raid immediately on enrage.
When we were learning him in guild, we did initially kill him few seconds after enrage. He usually takes a couple of seconds to kill 2 or 3 people, but depending on his hp, luck and cooldowns used, you may kill him little after he's enraged.
Also, a lot of classes do higher damage than normal after boss is below 20 or so percent. For example warrior execute, and hunters have something special for that too, and paladins got something as well. So don't give up until you're dead - that's my policy.
I had several very close encounters on Emalon. I was MTing him, and everything is going fine, he's down to like 1% or something, and BAM! I'm dead, and boss dies right after. Turns out he hit me for 60k ... because he enraged.
Another time, we're fighting him and he's down to last few percent, so I hit my avoidance buttons, and see how he's not even hitting me, I'm not losing any hp at all. Than he dies, later on I posted webstats, and realized that he was about 10 seconds into enrage, and the fact that I survived was pure luck ... and avoidance trinket.
All these close calls were probably because we used 3 tanks: me (dk) on boss, paladin on adds, and warrior to bring newly-spawned add to paladin tank. And we are very very very very (you get the point) "cautious", and bring a ton of healers.

Doin patchwerk for the first time (2 weeks after LK release), and patchy has 800 hp left. every1 is dead but me, and im running running running to the end of the room (towards the entrance) and BOOM! moonfire crit! killed it.
another epic kill was doing Malygos for the first time ( about 4 weeks after LK release), Maly enrages, every1 dies but me, hes got like... 2k hp left (cannot remember exactly, but a small amount like that). and me (the healer) killed it with a few hits while shielded :D
