I recently returned to my DK after leaving my main's raiding guild due to drama, and I'm trying to get back into tanking. I used to be pretty good.
For starters, this is me.
My gear isn't terrible, but I dont have anything from Ulduar yet. Mainly Naxx 10-25 gear that I had collected before 3.2 came out. As you can tell, it's been a while since I've tanked anything serious. Anyways, I came back and I'm finding myself severely lacking threat. I'm frost, and I don't exactly remember the rotation I used back in the day, but now I'm mainly just firing off Howling Blast when Frost Fever is about to expire (I have it glyphed), use Oblits whenever possible, and throw in Blood Strike (or Blood Boil for AoE pulls) to keep my blood runes on cool down. So I'm sure my rotation could use a bit of fine tuning.
Anyways, the guild I'm running with is mainly Ulduar 10-geared, so I'm thinking that it might simply be an issue that they out gear me too greatly and the dps can pull threat easily. Because of this, I'm considering changing some of my gems to gain additional Expertise to try and hit the cap. I'm not sure if this is the way to go or not though. Any thoughts or suggestions? Or is it simply that I'm out of practice and just need a bit of getting used to it?

For starters, get softcapped for hit (8%)
Even though you've got HB glyphed, you shouldn't use it for every occasion, I prefer using Icy Touch on single-targets and then turning the blood runes into death runes and using those to refresh Frost Fever. For AoE tanking I use D&D->HB->BB(x2 if BT is available), I've experienced that using only HB isn't enough for AoE tanking, and I need the initial D&D. After the initial pull, I spam HB whenever it's off CD, and spam Obliterate while praying for Rime procs.
I've only got 11 expertise (or something like that) at the moment, and I've tanked groups and raids with Ulduar geared characters doing dps.
If you're worried about initial aggro, use FS asap, as it'll always hit if you're softcapped for hit.

I am hit capped. I'm a draenei, so add 1% to my 7.08 =)

Try to replace the FS Glyph with D&D.
Although D&D is not doing really good damage for tank, but it has the greatest threat modifier for DK.
For single tanking, move that 3pts in Virulence to Scent of Blood. More RP means more Frost-strike, which helps a lot in single tanking.

Do they out gear you and is that a problem, absolutely!. They need to give you time to build threat.
However you need a more solid rotation. If you are AoE tanking than using HB on every rime and cooldown is a must in a group that out gears you. DnD is nice but glyphing it is not important, though I did it until my gear caught up with my raiding partners, than glyphed FS again.
DnD>HB>BB>FS
HB>OB>BB>BB
I am not a fan of BB. However I will use it on multiple targets when I am in a heavy AoE group. Most of the time I spend my BS or BB quickly so I can get Blade Barrier up. My guild is Ulduar 10 and my tanking gear is Naxx 10, I have to work my butt off to keep aggro, but having healers out gear you is nice except on pulls where adds spawn wher they get aggro before me is not nice

Plague Strike is a toss up. It shouldn't be needed, but it is okay for initial threat. On bosses I tend to use HB anyways because I don't like to vary my rotation, but Jim has a point. Sometimes Icy touch/PS on bosses is a great start, after the initial rotation though it is a significant loss in tps to continue to use PS, even to reapply your blood plague. 1OB is greater in TPS and DPS than IT/PS combo.
