I would do something like
this you get bloodworms, Imp Blood pres, Imp Blood Tap and Perma-ghoul. I left 5 points for you to put into something that you want.

Might like this.-gotta have that ghoul for added damage, chasing down half dead ppl, and his stun.
-spell deflection will save ur ass against mages in particular
-will of the necropolis is your pseudo cheat death talent
-you've got your worms and 1 point in imp blood presence for those auto self heals
-scent of blood will ensure you have RP to burn on those pesky runners
-glyphed heart strike to prevent those pesky runners
-glyphed IBF, just for the oh @#$% moments
-glyphed chains of ice for another ranged damage ability for any runners
Alternatively you may like
this for arenas.
-still has the ghoul with all his rotting wonder
-imped/glyphed rune tap, 10% health to your arena team every 30 seconds
-on a pale horse for CC reduction
-full blood worms and imp blood presence for max self healing
-spell deflection vs casters
-will of the necropolis because dying sucks
-glyph of blood tap, if the match goes quick enough horn of winter won't wear off and that health may just save your life
***If your not dead set on having worms (lol)
this may appeal to you as well.***
-Imp unholy presence so you can still chase ppl down while in blood presence for heals
-imp/glyphed rune tap for 10% arena team heals every 30 secs
-abomination's might so you still have a team buff
-still have the ghoul
-desolation because blood strike will be your main ability to spam
-death runes
-every sort of CC reduction (slows, stuns, fear)
-well you get the idea
