You are definitely geared to be tanking ICC and have enough expertise and hit that you should not be worried about getting more.
It sounds like you just have trigger-happy DPS.
If you are facing 3 or more mobs, throw faerie fire on one, tab and mangle the next and then tab and unload a maul. That will give you nice aggro on all three and then you can swipe/maul as needed.
You have 4T9 so don't be afraid to enrage in battle if you are really rage-starved, as you can barkskin with the short cool-down to negate the armor loss.

I primarily use Maul and Swipe on trash mobs (obviously as we have few options).
If people seem to focus a little on a target, I'll push a mangle or two onto that mob to hold them tighter.
If they don't focus and just aoe things down, Maul + Swipe is the way to go as you might lose them otherwise.
If they focus on a mob that is NOT your main target (watch out for Enhancement Shaman for this one) then get a Mangle on that target as dedicated dps on a target can still overpower your Maul + Swipe spam.
Often, especially in heroics, dps will jump the gun and attack because you're near the mobs. They don't care if you have rage enough to bat down a fly. You may need to ask them to hold up a little to make sure you have rage enough to start swiping before they go all out.

On my bear tank I usually just swipe+maul spam. If there is a marked mob, stay targeted on it so that your maul will always hit that one. If no marked mobs, tab while swiping. Watch your OMEN while doing this and if you see a DPS creeping up the threat table, mangle and FFF that one for a threat lead.

I started having the same problem as my gear increased in ilvl. I thought it was me, bad gemming or enchanting. It wasn't me, it was the dps. DPS (I'm guilty of it too) will see your gear score / health and figure that they can open up before you even get their while they might take it easier on a lesser geared tank. What I started doing was marking a skull, telling my dps that the skull dies first, i don't care what dies after. That gives you time to focus fire on skull and let your glyphed maul / swipe build aggro on the other mobs.
Edit: I almost forgot, I always whisper the healer and tell them that if the dps pulls not to heal them, it only takes once at 8G a death (for plate) to get the hint, you pull it, you tank it, you don't get heals.

Maby it´s just me but I like doing HC fast as hell and always try to do it faster and faster. So what if someone pulls agro. Grab it back whit a taunt. My gear isent all that greate but I don´t have any real problem whit threat. So what if some one dies in a HC if they are to triggerhappy. Remember it´s a game. Try chainpull if you fell you got the skills for it. Then you might get to the mobs faster then the others and get some Swipes in befor the rest comes and it´s greate fun too.
So my tip to you! Run faster and smash the buttons harder and faster ;P
Good luck whit your threat problems. Im sure you just need some more practice!

It's a problem with the class: swipe as snap AoE aggro is just so much worse than TC/D&D/Consecrate.
Except D&D and Consecrate are not snap tools either... lol...

Charge - Swipe x 2-3 - Demo Roar - Swipe xxxxxx - Demo Roar every once in a while. If I forget to put my maul glyph back in after raids I tend to lose one or two, but otherwise pretty much never.

Technically it's 2, swipe and maul. But you mentionned it in your second sentence, so I'll let you off the hook ;-)

I never understand bears with rage problems. You have enrage and a little bit of forward planning - like only white attacking the last mob of the pull - means you never run out of rage. I don't even use clear casting talent, I find it pretty pointless.
In 5-mans, sure you can't really get rage starved. Unless you overgear it massively, then you might run out of rage a tiny bit on single targets. Nothing a white hit without maul can't handle though.
In raids, you can get rage starved if you keep spamming maul while the other tank is getting the boss' hits.
Enrage should only be used if you have T10 4set, or in conjunction with barkskin. Unless it's a long walk. Even while massively overgearing, I do not like having less armor.
And lol at hitting the last mob with white attacks. I usually start running to the next trash pack while the last mob is still alive. He hits me in the back = no dodge = rage points, yum!

Bear tanks that severely outgear the instance, do indeed use kitty gear. The only thing I don't swap out personally, is my trinkets and my weapon (due to the berserking enchant).
Have both the macroed and unmacroed abilities on your bar for those low-rage situations. Unmacroed swipe works fine as well in hc's. But if you have excess rage, there's no reason NOT to use maul ;-)
