I hate the MT/OT idea because it gives the other tank a big head. Early into my ICC "career", I was "offtanking" and the Main tank kept taunting simple trash off of me "because he's the MT."
The ego it bestows is unnecessary.
I do that too. The other guy i tank with does it too. It might be a matter of ego, and it might not. When you get used to nameplates, and you see a nameplate that is not aggroed to you, it is a matter of reflex. I just click it and taunt it. I could check its target, and taunt accordingly to the information i would get, but that would be a waste of time. Some DPS could die by the time i decide to taunt. So often when i see nameplates that are not red and i do not know who tanks them, i instantly taunt the monster back, even if it was the other tank who naturally pulled off me.
DPS can compete with each other. Tanks can not. Competition between tanks will always have negative results on the raids success. Tanks should cooperate and respect each other.
In end-game raiding, the role of an "Off Tank" is greatly distorted. In the past raids, it might be fleshed out better, there might be certain things OT is a specialist for, and some other thing the MT is a specialist for. I do not know, really. Did not raid pre-WotLK. But in the present raids, and mostly ICC... you need two "Main Tanks". They should both be geared adequately for the content they are planning to tank, and they should both be able to do all the tasks that the raid demands of them.
There are tons of bosses, that require tank switches. On those bosses, both tanks need to be able to survive the beating. You will not beat heroic Saurfang or Sindragosa with an "Off Tank", that is not specced correctly, or geared enough.
There are some bosses that require one tank to take the boss, and the other to do something else. Both tanks in the raid should be able to complete both tasks as good as the other, but generally, tanking the boss is simpler, and doing the second task is harder, so the more skilled tank takes the more complicated job.
There are some bosses that can be done with a single tank... Lady Deathwhisper, Valithria, the Lich King... on those, the other tank should go DPS, and the tank that can do more DPS should be the one that offspecs.
(example? When we do LDW, it is either me, or the pally who tanks the boss. We are both able to tank the boss. The paladin can do about 12k dps in his retribution spec, and i can do about 10k dps in my balance spec <not on LDW, meh, we measured those values on Saurfang... point is, he does more damage>, so he goes DPS, and i tank. There is nothing that makes me more "main" than him)
Lastly, there are people who use the term "Off Tank" as an universal
excuse
for their wrong talent specs, or inferior gear. They should die in a fire.
All in all, i think that the term "off tank" is old, and nothing like an off tank is needed in the current raiding. You need two full blood tanks, both capable and geared. If anything, the term "off tank" should hint towards someones bad skill, or inferior equipment.

I guess that's a bonus of being a paladin. I glance at grid for non-tank aggro, and click for RD. I imagine you'd get a bit more trigger happy as a different class.

This can be done with other bosses too. Some from Icecrown are Marrowgar, add tanking Gunship, and if you're good at multitasking, purple orb tanking in Blood Council.
