I'm pretty sure I've done this before, but heck, why not?
These are the ones I currently play actively, and aren't basically taking up room on my server.
Owlwitch (hunter): I love owls, and I got called a witch a lot as a child (Yay being different in a small town!), so I took ownership of the term.
Darkwytch (shaman): She has the darkest Draenei skin tone, so I thought it fitting.
Lightwitch (priest): Palest Draenei skin tone, white hair, and priestly.
Wikkidwitch (DK): Because it was fun
Phoenixwitch (BE lock): I love the Blood Elf phoenix, so there you go
Moonwitch (druid): She's a Night Elf Boom, it fit.
Shadowwitch (rogue): She's always lurking in the shadows
Sandwytch (Panda Monk): As soon as I heard about the Epicurean buff, I knew I had to have this as her name.

Malavice: Mal = bad, Vice = addiction. Only later did I find out the name was used in Dr. Who.
Keratoma: Named after my cousin's friend's band.
Shunt: Was watching old replays of Robot Wars and was surprised to find the name available.
Crankshot: Simply can't think of a better name for an engineering goblin hunter.
Phael: Probably an overused variation of Fail, but meh.
Sunuva: Read "Son of a..." He was a retadin while I was leveling him.
Gaiarashu: My shaman, whose name is a portmanteau of the names of 3 elemental gods.
Demonburger: Translation of Dimmu Borgir, the heavy metal band.
Blood Elf DK - Helvete:
compound word from old Norse Hel (Hell) Vitte (punishment) so Hell's punishment, It can also mean Hell's wrath.

And about the three Tams ... I have bad imagination, and they are RPwise two twins and their mother, so their names are very similar.

I name my characters out of what I think sounds cool or fits the race/gender/class.
My Paladin was going to be a "name-play" together with my brother, he was Izyx and I just reversed it, Xyzi. We had some fun with friends when they tried to call the right brother and we corrected them.
When I transfered to another server the name was taken, and I went to an old name I used on another paladin in another game (ranged class, because paladins are cut for non-melee action right?).
I found inspiration from Tolkien's Silmarillion, When looking on the races and names I found about the elves. They are called calaquendi (Light-folk or smthing), Moroquendi (dark-folk) and there might be something else I have forgotten.
I took the liberty and named my character Moroquendin with the thought of having a "dark elf" as a character even if the grammar is wrong. Being a Blood elf makes the name so much more "accurate" for what we are as a race. Maybe it was a stupid call... but I could never wash away the name of Xyzi from my paladin, and I'm back to where I started.

My orc warriors name is "Brödrostrost" Brödrost means a toaster (you know the kind you put sliced bread into) in my language, and the second "rost" means rust. Rust in this case can also refer to the small burned out crumbs that accumulate in the bottom of the toaster. So my name can either mean "the rust on a toaster" or "toaster crumbs".
My hunters name is "Grönmögelost" Which is a type of bluecheese, but green. My dks name is "Kajsahelene" which is named after my wife.
I choose these types of names because i suffer from a severe case of limited imagination. My forum name is a type of candy, with a chemical fake banana flavor, some types dipped in chokolate, shaped like small bananas.

Akitsu-Belf DK-anime name (Sekirei)

Rammingspeed- troll druid- from the line in Army of Darkness( which in turn is in homage to Animal House)
Uzume-Belf Pally- again, Sekirei name.

"Calaelen" is what some random Elvish name generator said my last name was in Elvish, and it somehow stuck all these years. For various reasons I ended up with a toon called "MasterCalaelen" in the game PlanetSide, which ended up in everyone calling me "Masta" on Ventrilo. I'm part of a group that's called Delta Triad, which is abbreviated to "DT", and since that group originall all played WoW, most of us had "DT" at the end of our names. Nearly all my names can be led back to the above:
Mastadt (Mage) - obvious
Nelealac (DK) - Calaelen backwards
Nelstadt (Paladin) - Combination of my Mage and DK's name,
Nel
ealac and Ma
stadt
Macael (Hunter) - Combination of Mastadt and Calaelen
Mastac (Druid) - My ventrilo name "Masta" + the "C" from "Calaelen"
My Warrior and Rogue are the only two that have random names:
Prikker (Rogue) - Dutch for "stabber", which I found suiting for a Rogue
Raepskrad (Warrior) - "Darkspear" backwards, he's a Troll and I couldn't think of anything clever at the time so I went with that.

Rystrave I made up while playing FF.... 9? Or 8? I dunno, whichever one where you can name your airship. I've stuck with it since. Scarns is a combo of my first and last name.

Höwnöw - tauren druid: 'cause she's a brown cow. I had to add the accent marks after a server change.
Friesian - tauren shaman: after the breed name, 'cause Holstein was taken.
Gyushiro - tauren paladin: "white cow" in Japanese.
Mazrigal - orc rogue: in a fit of creativity I actually went for a unique name. I think I'm still the only one who's used it so far.
Pocketpistol - goblin hunter: 'cause she's short and uses guns, get it?
Lebewohl - orc death knight: German "farewell".
Chikung - pandaren priest: alternate spelling of Qigong, Chinese meaning "life energy cultivation", a type of healing art.
Huohuli - pandaren warrior: approximation of "burning fox" in Chinese. I liked it because some of the red skins for pandas actually made them look much more like a fox than a red panda. Then I found out they made some of the minor glyphs make warriors leave trails of flame when they charge and make them spontaneously combust when they get an enrage effect and liked it even more.
about the username: First character I ever created was a gnome mage named Tockley on a friend's account, a name I thought up and liked the sound of. It was a long time later that a nice Australian person on the server pointed out that it was a slang term for penis there. I'm not sure why, but I've kept using it as an online handle (eheh), because I find it funny despite not usually being a fan of lewd references.
