Why they didn't die is because apparently in the WoWverse, undead do not die when the necromancer that raised them perishes. Kel'thuzad's undead (even the non-plague ones) survived after Arthas killed him, and the Forsaken stayed animated after they broke away.
You might think of it like this: there are two types of magic that could be considered "necromancy."
In one type, the spellcaster literally animates the dead flesh, as if he were playing with a puppet. The magic that makes the construct move is entirely supplied in real time by the necromancer, and when cut off from the magic, the contruct will fall apart. This would be the most elementary form of necromancy, and it is incredibly inefficient in terms of magical power.
In the second type, however, the necromancer provides a small amount of "ignition" magic, which then essentially turns the contruct into an independent necromantic "engine," supplying their own "power" and remaining animated even when vast distances or death prevent the necromancer from supplying them with further magic. The necromancer can then control these independent constructs with mind-control, rather than brute force puppeteering. This is remarkably efficient because not only can the constructs remain independent and effective without the necromancer having to stay focused on them 100% of the time, but in addition, contructs with a high necromantic magic "output" can in turn "ignite" the spark in other constructs, which facilitates such things as Death Knights and Val'kyr being able to raise undead of their own. If they were required to be supplied with constant power from their original animator, then you end up with a situation where the original necromancer's power gets spread successively thinner as he raises death knights who raise undead. It's this system that the Lich King has apparently opted to use, and that's why Death Knights and Forsaken can break away and remain animated, and zombies could theoretically live after his death.
As for why they didn't go free when Bolvar took the throne, that's because he has become the new Lich King, and gained all of the powers that came with the job.
And as for why they don't just have Bolvar gather up the undead and burn them to ashes, we don't know that. It seems like a simple enough plan.
It's possible that there is still an evil entity in the Helm of Domination (namely, the Lich King; we may have seen a glimpse of good Arthas after the helm fell off and Frostmourne broke, but that doesn't mean that the
thing
that was created when Arthas and Ner'zhul fused together is completely out of the picture), and that that entity is fighting with Bolvar for control of the Scourge, and that it's all Bolvar can do to just keep them reined in. But, again, this is only speculation. We don't know what the answer is yet, but Blizzard and Metzen are creative, and I'm sure they'll come up with something.

It could be worse.
It was all a setback so we have a faction to gain rep towards in a later expansion.
Heh, indeed. And I do think it's that simple Skree; I mean, they've never explained where this 'sudden' rampaging horde of Scourge would come from that would eradicate the world; even those who died in ICC wouldn't be enough to destroy all of the living in Azeroth. Plus you know....we spent two expansions destroying the Scourge forces /sigh
It would have been a lot better to just have had the remnants of the Scourge only a problem in Icecrown, which would give the Crusade and the Blade a reason to still exist.

We still have Blood Princes Theraldis and Atherann MIA, and considering the fact that it was Theraldis who was commanding the other princes during resurrection of Arugal I think that he is more than able to rally quite a formidable army under his command. Plus, Mal'Ganis's whereabouts are unknown either, and who knows with whom he may side now.
In other words, there are still so many possibilities for the undead-themed dungeons.
