Cyclone/Roots/Chains of Ice on rogue, rape healer. Also, do try to keep dots on both targets, it will rape healer mana.

Death Grip the healer, forcing him onto the defensive when the rogue is vanished?

You dont always have to kill the healer first. I have seen this many times. If you are good at key binding you can mouse over macro your silence ability. Also sounds like you need to learn your character better. With icebound fortitude that rogue really cant stun lock you if you pop Anti magic shield right after. Your healer should be CCing the healer as well. There is no way that rogue is tearing you down so fast that your healer cant doing anything else besides spam heals on you. UH DK and resto druid are awesome combo.
You are unholy. Sick your pet on the healer and after he transforms you have like a 8 second long stun lock on the healer with just your pet alone. That rogue should be running from you.

You can try to go behind a pillar or box with your healer and work down the rogue. Try to keep the rogue locked down with you behind the pillar or box, so the rogue's healer is forced to come to you.
If the rogue gets away with a vanish, hard swap to the healer and work the healer down, the rogue will probably be forced to come in and peel you off. At that moment, try to bring the rogue out of LoS of his healer who is probably fleeing and work him back down. If the rogue's healer ever tries to rescue the rogue by coming to his aid, cyclone him.
I haven't played a druid, rogue, shaman, or DK. But i would think this strat would work.
