The exposed head and Magmaw are two different bosses, they share 2 sets of threat if im not mistaken.
You are correct.
The tank has to switch targets and start gaining threat from it.
Taunting won't work just gain threat by dpsing it.

The tank can attack the head while being chomped on. If he isn't doing that then yeah, you will have threat problems from spam healing him.
Remember to bring some
Potions of Concentration
for the burn phase!

Do you have 2 tanks? I assume you are talking about 10 man. I do not think this is avoidable if you have 1 tank. I may be wrong since I don't do 10 mans anymore :\
Another possibility, if you're a well organized group is to have everyone but the tank move slightly out of melee range while the head is down. Then, when it comes back up, the only possible target is the tank. At that point you have a short window to move back into melee range before the next pillar.
Ober has your fix. We only did him once on 10 man before the guild had enough peeps for 25 but Ober's fix is what we used.

thanks guys, well try moving everybody out of melee at the end of P2. I appreciate the help
JUST out of melee and immediately back in the second the tank has him picked up or the 'Pillar' is going to be intimate with your melee.

It's not your fault. It's your tanks fault.
I tank this fight on my pally with a resto druid healing in melee range. When Magmaw lifts his head back up after being in the prone position the tank needs to select him and taunt.
Essentially prone Magmaw and upright Magmaw are two different bosses. The tank will lose target during the shift. That is why he needs to reselect him. If for some reason your tank is one of those that rides the short bus, have him make a target/taunt macro.
ACTUALLY what you are seeing is that Magmaw's next threat target is one of the people still at range when he comes up. You are not the target, as such you don't get gibbed.
