The rotation I use on single target is "Open with Garrote > SnD > Mut x2 > Rupture > Mut x2 > Envenom
The mutilate rotation has never been rigid and varies according to ruthlessness procs. Read the stickies.

When I made the simple change in thinking from "Never let Rupture fall off" to "Never let Envenom fall off", my DPS jumped heavily.
As for the rest of the "reforge", "re-enchant" stuff, take the advice given here and EJ. Four changes last week (re-reforged, re-enchanted, AND re-gemmed, along with my change in rotation) saw me jump from 10-12k DPS raiding, to over 15k.
One piece of advice about gems: Purple and Orange. Orange always, Purple if you have the Hit to spare. Both of these colors give you Agi, which is the GOD STAT for Rogues, so the more you get, the better!

Mutilate rotation doesn't exist.
A priority chart does exist though, and it is snd > rupture (at any combo) > envenom. this means you want to refresh your rupture as late as you can (if same combo point, you can refresh it when it has less than 3sec left) and use envenom to refresh snd without clipping the bonus.
As opener, you can do whatever suites you better, anyway i do garrote>rupture>snd / muti>snd (depending on ruth proc) >mut >envenom.

Actually due to venomous wounds only proccing off of either garrote or rupture not both, your starter rotation should be garrote, snd, mut, envenom, vendetta, mutx2,rupture, then keep up envenom buff and rupture without clipping. Of course if there is a burst part of the fight, save your vendetta, vanish and other offensive cooldowns for that.
Wrong, Venomous Wounds can proc off of Rupture and Garrote at the same time.
