I don't do anything differently at all. Pop RF and then Ais > SS > SS > AiS.
Since the MD effect won't begin until you finish casting the first AiS, you will have time for the 2x SS and 2nd Ais before it fades.

Misdirection --> RF Aimed, Chimera, Double Steady, Serp (because MD is down by then anyways. This is assuming the mob is not at Careful Aim level. Not sure what to do there), do normal rotation.

I think you'll have enough focus with RF up.
Double MD is kinda overkill though.

I don't think that's true. With the ridiculous amount of threat that an MM Hunter generates during the first 30 seconds, i will pull threat off the tank every single time if i don't MD. I don't think it's the tanks fault at all.
Anyway, i still use my optimal opening rotation which is RF > AiS > SS > SS > AiS. The reason this is optimal is b/c it immediately dumps some focus with AiS so that you aren't sitting there capped, and then does the next most important thing which is get your ISS buff up. If you open with RF > AiS > AiS > SS > SS you waste approx .2 seconds on your 2nd AiS b/c ISS isn't up yet. And your 2nd AiS will still get MDd either way so nothing changes there.
