Mouse-over target is when you mouse-over someone and your tooltip pops up. Usually mousing the raidframes.

Let's say you want to use
Misdirection
on the tank without having to select it (or unselect your target/boss/mob...)
/cast Misdirection;
If you have your tank on focus (/focus) and you are targeting the boss, the following macro will have the same effect as the first with @mouseover.
/cast Misdirection;
That was my example.

The standard healing macro goes something like this...
/cast Heal
Priority falls through from your friendly, living
mouseover
, to your friendly, living
target
to you.

You can't click a mouseover macro, as your action bars aren't a valid unit to cast spells on. You have to use a mouseover macro with a keybind. Otherwise, what you said is true. Whatever unit is under your cursor will receive the spell.
I'm not sure how you bind something to left-click...?

Can you even bind
Mouse Button 1
to anything? You'd need some special driver that allows you to remap your mouse's buttons to do so. As far as I know,
Mouse Button 1-2
can not be bound to something. Any other
Mouse Button
would work though.
