Not necessarily; the tank could have gotten placed in another group that was before yours where the original tank declined or timed out.

In general though, once your queue pops you should be at the front of the line. There are numerous behind the scenes things going on, but you should still be at/near the front of the line.
To answer the implied question, you are not sent to the back of the line if someone else declines the group invite.

If your queue time is 20 min plus, and you end up with it before that point, you are probably looking at a in progress instance; that's something I've noticed a few times over the last few months. There is some randomness to the queues though, as I've had it before where it popped up at the 15 min mark, had someone decline, and didn't end up with the next chance till the 25 min mark instead.

And if you kick your group mates and reque, it will stick you with them again. Same thing if you get kicked and reque.

Also take into consideration that the LFD system tries to match you with a certain set of classes.
This is why it is rare to get 2 of the same class in LFD, if you are a Deathknight and you leave your group, LFD will try to replace that DK with a class that can fulfill the same roles (aside from the obvious dps) such as interrupting, slowing and or stunning.
I've tanked around 280 5 mans and 90% of the time the class that leaves gets replaced by the same class.
This is especially important in dungeons like Zul'Aman, because of Hex Lord Malacrass. It's entirely possible to not be able to kill him based on your team composition. What if you can't CC the adds? What if you can't interrupt the adds spells? What if you can't interrupt his and the adds spells?
That's why a balanced team is required for most dungeons. You can't go all guns blazing with 5 Warriors. Regardless of how awesome that would be
