Wow, you're coming down pretty hard on that guy, Fishdert. Obviously you spend a large portion of your game time with people that don't know their class. Hunters (or, more specifically, their pets) can indeed tank 5-mans. I've completed regular Sethekk Halls with 2 hunters with their pets tanking, 2 more DPS, and a healer.
Sure, most PuGs won't have 5 competent people playing, but when you get some nice teamwork going, it can be a lot of fun.

this was in Ulda, i dont know how we survived but we killed everything but that final boss.. before patch lowered his lvl.

You know, it's pretty hard to make an instance group in WoW which fails because of class composition. If you're doing an instance at the right level, there are only a couple of real requirements:
- a tank (war, druid, pala) who is active, works at keeping aggro and knows how to pull
- a healer (priest, druid, shammy, pala) who actually heals, pays attention and avoids getting killed
- 3x dps who give the tank time to establish aggro and stop dps if they steal it
And even then I've been in good groups where a Voidwalker has tanked (lock specc'ed with enh voidie) or a hunter pet has tanked. Those need good handling of CC, and more control and patience from the players, but it can be done, tho usually with a few over-levels.
If either your tank or your healer slack, you're in trouble, but the dps being less than great doesn't matter as much, as long as they do some damage and don't get themselves killed. Also, if the warrior or pala tank doesn't use 1H+shield it's always a bad sign, as is a druid who says they can tank in catform (unless they're 10+ levels above the dungeon's level).
It sounds like your Deadmines problems are a combination of being slightly under-level (a lvl 21 group with mostly inexperienced players will have a few issues towards the end) and the tank not pulling mobs back far enough from the other mobs in the rooms. Keep in mind that even mobs with ranged attacks will follow you if you get far enough back... The patrols there are always a pain at the right level though, they show up at set places and it's best to be prepared.

Hunters can indeed tank. When I leveled my hunter, I refused to bring a warrior into ZF because to many wanted to either MS or Fury tank ( and fail at it) or just sucked with sword/board (or had an excuse why they didn't have one).
Let's qualify that.
Some
hunters can tank, if the pet has enough armour and the player is good enough. I wouldn't recommend it anywhere with big group pulls though. In ZF you can probably manage, but BRD or BRS would be a whole different ballgame...
On the whole, Druids and Warriors are better equipped, and truth be told Druid is probably the easiest tanking class, even though Warriors have more all-around tools.
Paladins can be decent tanks anytime post lvl 16, which is when they get
Righteous Fury
and
Righteous Defense
... they just have to know how to play, you should expect any first-time tank to have some trouble.

Also maybe I just hadn't noticed before but I thought most enemies wouldn't swim after you. Previously I could escape many enemies by hopping in the water and swimming down and away, but recently I've had goblins, humans, and orc NPC's swim after me and finish me off.
Oh they will swim after you. Can't think of many that won't (Moam in AQ20 maybe?). More of a problem is for fish and some other underwater creatures to follow you out of the water. They will typically start evade-bugging if you go out of the water.
And the big problem with swimming is that the mobs almost always swim faster than you do. So water is a bad place to run away from mobs.

I actually completed SM with this O_o (Pets were tanking).
I've been in similar groups, and it's actually not too bad, because you've got multiple Voidie tanks, maybe some Succubus CC, frost nova and polymorph from the mage, so lots of control options and as long as people use what they have well, it normally is all fine.

Were you just trying to be funny? You have a tank, healer, and 3 DPS. How is that even a contender for worst pug? 5 Blessings, 5 Auras, 5 DIs...
The other day I got myself involved in a PuG heroic Slave Pens. 3 Druids and 2 Warlocks. We did
not
use succubus CC. We had a couple partial wipes that we recovered from with the battle rezzes and soul stone. We only actually had to run back once (when we pulled all 7 of the bogstroks in Quag's room).
Interesting group setups can make running an instance enjoyable, as long as the players in the group are reasonably good at their classes.
