Are you on a limited/power user account on windows XP without write permissions to the wow folder?

Vista is a whole problem on it's own.
In short, yes the issue is that wow does not have write permissions to the folder it's in.
There's 2 possible causes for this.
1. You have wow in the default install location (C:\Program Files\). In vista this is a "special" directory, and things running from it typically do not have permission to create/edit files in this folder. Instead, it redirects all the files to another directory (wow doesn't like this, and can't find the files).
To solve this, just move your entire wow directory out of your program files directory to some place else (like the root of your C:\ drive... ie. C:\World of Warcraft). This won't effect the game at all (wow is designed to run anywhere, it doesn't need to be installed per say, you can actually just copy the folder to install it XD). What this does do is get vista to stop redirecting the file creation.
2. You do not have write permissions on the folder (make sure you try option 1 first), this should only be the case if you're not an admin/power-user.
