Exactly how to make a death knight?

If memory serves me correct, all you need is a level 55 character or higher in order to unlock the Death Knight. You can only create a Death Knight on a realm in which you have a level 55 character or higher. In addition to what I previously mentioned, just like it would normally be on a PvP realm, you cannot create a Death Knight of the opposite faction (i.e. if you have a Horde character on a PvP realm, you cannot create an Alliance Death Knight, and vice versa).

The only addition I'd make is that Death Knights are unlocked account-wide rather then server wide. Otherwise you are absolutely correct.
