Offhand that should work just as you want, the only change I would make is to move the clearfocus up a line to smooth out the sequence. Also if you want to auto attack can add a /startattack
keep in mind this can be dangerous if you end up hitting a mob before it's ready. It's safer if you're melee, dangerous if a hunter.
but it would look like:
/focus
/clearfocus
/target focustarget
/startattack

You're going to want a stopmacro in there. Also, you can use assist with focus and it will then respect your interface setting WRT "attack on assist". I'd go this way:
/focus
/clearfocus
/stopmacro
/assist focus

You can make a polymorph one by changing Counterspell to Polymorph (Rank 4), same with any other useful spells you might have.
Not too big a deal as we'll lose all spell ranks in a few months, but don't ever specify the rank of a spell in a macro unless you mean to cast something other than max rank.
/cast Polymorph
That will always cast the highest rank.
