Pretty much like this, though usually with the gryphons and background on the action bars. It's a modified IboUI and a bunch of macros and scripts.
OOCI got tired of some quirky things and settled on a simpler version. The spell history over where focus, target, self, and party members should be comes from TrufiGCD.

Toying with some mockups for a new UI. I like RealUI's unit frames a lot and wanted to incorporate something similar. I worked off the basic idea as a template and developed this mockup:

The purple/orange is class-coloring. Player's class-color indicator is red in combat. The dashes along the target hp bar (to the left) denote combo points; a similar scheme will denote runes/shards/etc. The icons to the side of the player frame are indicators for shields (top, number in brackets is duration left); and crowd-control (number is duration left; color is red for CC, white for silenced, school-colored for interrupted). The indicators will be present for all units.

This is where I'm at creatively. It's still just a Photoshop mockup. I'm not sure if I like the unit frames to be honest -- the slanted look nags me, but I don't know why. I'm gonna toy around with ideas a lot more. So of course feedback is appreciated.
http://i48.tinypic.com/j982sn.pngThe auras are one of the functionality ideas I came up with that I'm definitely sticking to. "Emphasized" auras are shown large and first, with the rest shown small. This way important effects (my bleeds/HoTs on enemies, how long until a Frost Bomb ignites on me, Warrior TFB stacks, etc) can be easily tracked without having the entire aura area oversized. (Party/arena frames will show emphasized effects only.)

My UII wanted to keep as much of the default Blizzard style as possible which is why I went with Blizzard style bars and font.

As I see, the main issue is, that while Grid is the most important, it's too small to properly use additional indicators (like raid debuffs, etc). I just can't see which debuff is applied, because of the timer, but I think it should be there. :S
The second is, that I think my action bar is a mess, it's pretty much a legacy as it looks since like 2009.
I have two chat windows, in the left one (as you can see) are the standard messages (General, Guild, say/yell/emote etc). Basically the ones that can be ignored. In the right one, there is the instance/party/raid chat, as well as the whispers. I just hate it when unimportant chit chat suppresses things I should've read, so that's why :D

Although UI's are a more personal thing, I can tell you what I think about it.
First off, I'll let you look at mine (
http://i.imgur.com/TsFCoKz.jpg), its just Elv UI with my own changes to it (I used to make my own UI, but got lazy over time)
As you can see my bars are two small rows, with two big rows on either side. The rows on the side are the ones that are not keybinded, so I've made them larger to be able to click on them easily.
The two rows in the middle are all keybinded, so they have been reduced in size.
Why I am mentioning this is because, your bars are taking up a lot of space, because of their size, and space between each button; which is quite unnecessary and causes clutter.
Also you can skin the looks of your action buttons as well as your buff icons. using an addon called Masque (

Next would be your two chat frames, as you can see I only have one, but if you may have noticed, I have multiple chat tabs (Whisper/Party/Guild), and if you look closely, the Whisper tab has lit up, saying that I have a whisper that I haven't read yet. This keeps having a 2nd chat frame open, wasting significant space on my UI, which ive used for Skada frames, Threat and Damage.
The next part would be your healing frames. I know this is crucial, since I was a main spec healer for 3 years before I went tank. and yes, I agree that GRID is too small. frames like that are mostly used for non-healers just so they have some kind of indication of whats happening with the raid.
A lot of healers I know go for something a bit more chunky, bigger frames, more spacing for debuff icons and HoT timers etc.
I personally haven't used Grid before, but I assume that going through some options you can enlarge the frame. As for the position of Grid, thats up to you. but it seems like you have a perfect spot for it on the right-middle side of your screen above the right chat frame.
Other alternatives for Grid would be Vuhdo or Healbot, they will work the same as grid, using your mouse-over macros, but will also allow you to bind your Left and Right mouse clicks to extra abilities, which does save bar space as well. Though these two addons need a lot of time to configure them properly so that they perform well, and look good. Default profiles for them aren't great.
