Mousewheels has no keydown-keyup like all other keys has, it's only keypress.
To make a keypress into a modifier you'd have some other application to record the wheel and then your second key, and generate another keypress based on which the second key is.
What you can do though is bind whatever you want to your wheel. For example, you can bind potions to the wheel, and then modifiers change what they do.
Scrolling in and out could, for example, be alt-scroll.
On a sidenote, do you like your new naga? I have JUST ordered one, and it's on it's way here. Should have it tomorrow.

It seems you can do this in the ordinary razorapplication aswell (i just got my naga).
Although, I like having it as numpad. That way, I can first bind all numpads to actions, then ctrl-numpads to other actions, and shift, and alt. (that is, ctrl/alt/shift+nagabutton, giving me a total of 48 buttons, just on the mouse)
