Level 70 DK tank, dying like a hooker in 1888 London

I'm trying to prevent this from being a "QQ heres my armor fix me," but it's rather difficult. I've been interested in a tanking class for a while now, and have finally gotten my dk up to where tanking requires a bit of specialization. During BC I could tank in whatever gear I had laying around, but for Northrend I went out and bought some pieces of cobalt. This, I figured, would probably be more than enough to survive the first several instances of Wrath. However, I have yet to complete Utgarde Keep, not for lack of trying, and not because I lose threat.
Straight up, I die disgustingly fast. 4-5 seconds without CDs on trash pulls of 3 or 4 mobs. I have a healer friend and a tank friend who both say that what is happening is not my fault, and that healers should be doing better, but I can't help but think I have made some mistake somewhere.
Armory Link:
http://www.wowarmory.com/character-sheet.xml?r=Skullcrusher&cn=Longdeadtank
It seems to me that my 45%+ avoidance and damage reduction of 62% should be enough to tank Utgarde, but every group I have had now has not been able to clear past Prince Kelseth.
Any replies about what to do/fix in regards to survivability are greatly welcomed. I would like to start using the LFG system more now, so I get more comfortable with my dk before raiding/heroics.
Thanks a bunch.

You dont need Icy Talyons, or imp Icy talyons for that matter,
just the 3 pts in improved touch, thats going to free up 6 points for you to use,
max out Gorefiend, - helps your UB
- Dont be affaid to use UB, you should use it as much as you can
thats the only things i can find right now, maybe want hungering cold to freez mobs to help reduce dmg,
Didnt look at ur gear b/c thats always changing,

Um... you're DW specced but you only have one weapon equipped?
That said, I'd probably go 2H until you get a bit higher. It'll free up a lot of talent points for other things.
You might also want to consider glyphing for survival (UA or IBF or both) for some extra oomph on your CDs.

Also, get your skills up. Mining will increase your Stamina not insiginificantly, and Blacksmithing would let you socket some extra defense gems.

You really don't need defense until 80 heroics. At which point you need 535 to be def capped. Until then get stamina for the extra comfort on healers.
Edit: if you have extra gold to spare get CHEAP stamina gems, if not it's not a big deal since your gear will be constantly upgraded. I myself didn't gem or enchant blues, but went all out on my epics. Gems, enchants, everything.

Glyph of IBF is purely PvP glyph. By glyphing IBF in your tank gear and tank spec, you actually gimp it instead of boosting.

You dont need defence capped, but certainly need any defence/dodge/parry he can get on gear. Gem idd for sta.

Only switch to Dual wield Frost when you have oodles of +Hit and nowhere to put it. If you are having trouble, stick to 2H Frost and use the talents elsewhere like the poster above said.
Enchant your stuff! Borean armor kit = +12 STA. Heavy Borean Armor kit = +18 STA.
If you have money to burn, then start slapping those babies on your gear for another 60 to 90 STA. That's another 1000 health at level 70 with five armor kits. HUGE boost.
Dual wield? Yea, that's kinda gimping you.
Get the Quest drop 2Hander from Borean Tundra.
Hellscream's Champion
. Specifically, it's the best 2H axe you'll see till level 75 whend you get the Ampitheater of Anguish quest. Did I mention this 2H axe is just gods awesome and obtainable at your level? Throw a Rune on that and go to town.
Did the quest on the Alliance side three times for the same axe. Warrior, Pally, DK. All three classes benefit from it, unless you're using a Tier 6.5 drop from a high end BC raid. Otherwise, get that axe. It's super easy.
Stat food. I have three level 80's that have maxxed out cooking. I ship stat food out to all my alts and send some to my son's characters. Constantly having 30 Stam and 30 Crit is nice. Potions help a bit and are still cheap.
I'm not saying use Stoneblood Flasks for Utgarde Keep, just bulk up a bit with some easy consumables.
I have a grand total of 150g across two 80s at the moment. Lent my friend gold so he could buy dual spec.
That quest is actually in my log, but at the same time, I have a feeling that a single weapon is not about to fix the fact that I'm getting chopped up.
Also, thanks for the advice everybody. I kind of feel like it is just a matter that Utgarde is just Utgarde, but I wanted to make sure I wasn't overlooking anything. I used the guide that's in this forum for my spec, and I have the 1h rather than my 2h heirloom because my parry and dodge are higher with the one hander, and my health drops by just 250 something.

Utgarde keep first thrash mobs use some kind of
stun/disarm
/
sunder/enrage
combo which can make even geared tank instagibbed (cause it removes parry out of the equation) if the healer isnt good, so keep trying.

Get HB glyph (replace RS), and keep FF up on all mobs, always. Almost 20% less damage.
but every group I have had now has not been able to clear past Prince Kelseth.Be sure the dps know to destroy the ice blocks, and that you aoe the skellies. And on the last boss, run away from his big hits.

And an update for whoever cares, it was the healers. Got a level 72 druid earlier tonight and we got through without a single death. My confidence is now restored. Thanks for all the advice guys.

This is true at 80 because it makes IBF function as if you had maxed out Defense even if you don't (which all tanks at 80 will). However, at 70, you won't have the defense to max out IBF, generally speaking, so it would actually make the ability much more effective.
(Unless the defense required scales with level. I couldn't find anything to suggest that it does.)
