One thing I might mention about ICC is that it is producing an order of magnitude more combat log entries than any of the other previous raids of Wrath. A full night of raiding in Ulduar was producing a combat log of maybe 90-120 megabytes. A night of ICC is producing a combat log of 200+ megabytes. All those extra events have to be filtered through all the addons you have that process them. More processing, more lines, more work, more lag. ICC is very hard on people's computers.

A disabled addon won't even load its saved variables. Even if you do have it enabled, it will not incur a performance hit except on reloads, zone changes (maybe) and when it requires a lookup (i.e. you hover an item and it has to look up the info).

To me on my crappy computer, I feel that it's the addons that need to constantly be updated that slow down the game.
I used to use pitbull 3, and recently got rid of it in favor of grid for healing and it really helped my fps. I also got
pMinimap
in favor of sexy map. I use
Sage
for my player and target frames. It's not very configurable, and when I say not very, you really cant configure anything. But it looks a lot like the standard pitbull frames and does the job well enough. Dropped quartz for
AZcastbar
...not as pretty but does the same stuff.
Like everyone above said...grid mana bars is a hog...so i dunno if you really need to be seeing everyone's mana. You could always let someone in vent tell you if raid mana is low and you should pop divine hymn or whatever it's called.
Also, turn them portraits off. They always slowed my computer down.
