Keratoma (3 others)
Malavice (1 other, and I know for a fact I was the original)
Protroast (reserved for my Tauren pally, but undoubtedly hundreds of these exist)

I've only ever came up with one (two, if you count my alt) unique name.
Shadowysmite, for my priest. Which then got 'copied' by my shaman alt, Shamanysmite.
Although, more than one person can 'invent' the same name.

I didn't have someone copy my name... but I have an archenemy in the BG's because of his name. My toon is Kennyisback. The name was based on Kenny from South Park who always dies at the end of the show and is always back (no questions asked) next episode. Since My toon is Undead, I figured it was perfect.
So I am in AV one day, and I run into: iKilledKenny. Needless to say I hunt him down every chance I get :-)

I got bored once and made a female alt named after my main (Khju). I was delighted to find that I had the only Khjugs on US servers. I've since deleted her though, so in this case I'm hopefully someone carries on the legacy, rather than upset that someone took the name.

I have 14 characters over level 10.
Of those...
3 have unique names on the US/Oceanic servers
3 have only one other character with a similar name
The remaining 8 have "cousins" with duplicate names ranging from 7 to 119.
Of the 14 names, one was a random number generation (7 total).
The rest are all "original creations", I suppose, especially with 8 of them having had characters in other games that pre-date WoW.

My first character ever (Nelf Prot warrior) is called Elezran.
Two months later I see an Orc Shaman with the exact same name in the armoury.
(Tempest Keep - EU)
I'm not really bugged by that, it just makes me wonder where the name comes from (if at all).

Most of my chars had an unique name at the time of creation; I generally check. Call it a minor obsession with being unique, but I can be quietly proud of the fact that I was the first regardless of how many more players using the same nick crop up. I don't mind either copies or people coming up with the same name later - I was the first at the time and I'm satisfied with that. Admittedly I don't even run into duplicates of my more obscure nicks often, something I at least partially put down to the effort gone into them, so I don't have to confront the issue much.
The chars that did not have unique names were never copies either - if I see someone with the nick I had in mind before I've made a character I avoid using it. Some names, like that of my rogue Nihilist, I
assumed
were already in use somewhere since it's quite an "obvious pick" but since I hadn't actually met or seen anyone using it I didn't mind using it anyway.
It annoys me when people name their characters after people known to WoW players, particularly famous PvPers that make movies. You are not and will likely never even be anything like the people who you copied the name of and merely end up demeaning yourself in the eyes of others with your lack of originality. Maybe that's just me, but not spending any effort to come up with a good name is just... I don't know, lacking in respect or enthusiasm for the character.
I dislike naming characters after real or fictional characters from other media (or real life) as well for similar reasons (although the distinction of awareness still applies to at least my own comfort; if you make a character with a certain name and a movie character in some big movie that comes out a year later has the same name I'd hardly be bothered), but I wouldn't go as far as to enforce that view on the people who don't and do as with the above.
You can't always know, though. My main alt in TBC was a paladin called Kirai - a name I had come up with myself, no inspiration from anywhere, it just sounded good to me. Then sometime in his mid-forties I get a whisper from some random asking if I'm japanese and I go "what". Turns out it means "hate" in japanese and I'm sharing it with about two trillion anime/manga fanboys. That fact has even spawned an Urban Dictionary entry (

) - I cringed a bit at the time but now I just find it mildly amusing and thankfully the number of random people asking about the nick and/or accusing me of one of the things with name picks I loathe was kept to below the minimum I expected after finding out.
