First post here, ready to be flamed. ;)
For those complaining about the tank explaining pulls, I'm annoyed by dps that move beyond the tank and aggro mobs, then complain when you don't have aggro and their pet is taunting away. Sometimes fights and tactics need to be explained to keep the bads in check.
For those complaining about the tank not being able to keep aggro, it's tough as a paladin when the dps doesn't even give time for consecration to hit. Be fair and give it a tick or two.
For those complaining that the tank runs in and aggros everything, sometimes it's because the dps stands in a bad spot and aggros stuff. I'm annoyed when the healer does nothing but look at health bars and can't see the hunter backing up into four dark iron dwarves and a hound patrol, then yells at me.
As far as people in LFD and tanks always dropping, it's usually a dps for me. I feel your pain and try to make runs as quick and painless as possible.
Healers that think they know everything in the world. I've had five 80s, tanks, dps and heals and didn't buy my account. How about you let me play the game and you can play yours?
People that leave on a wipe. The gold you've made far outnumbers what you've lost unless you've died 4+ times, so go make some bandages, sell them and recoup it. (Even as a prot warrior I never had money issues.)

As long as a DPS isn't pulling mobs by themselves, they're doing their job. No DPS should wait to enter combat unless the tank is undergeared. Hunters and rogues especially, due to tricks and misdirect.
Disagree. Paladins need a tick at least if 4 or more mobs are pulled. Hunters and rogues generally do not use their tricks. When's the last time you saw a hunter pull an instance? I could back in vanilla.

What annoys me?
-In battlegrounds people spamming /laugh, /spit, /rofl etc as they are attacking you. It's not cute, it's not funny. It's just plain immature
-People who purposely sit on quest NPCs, flightmasters, etc in mammoths, drakes or other lage mounts for the sole purpose of blocking other people from that NPC (however, in most cases having friendly healthbars up is a wonderful thing)
-The "Anal" game, the "Murloc" game, the "Last person to link

-GS abusers (It has its place but should not be the end all, be all for raid recruitment. When my guild has to pug we only pay attention to GS if its super low for a raid, like 3k for ICC. We still armory people, we've some across quite a few hunters with spirit enchants or gems O.o people with no enchants or gems. GS doesn't catch these things)
-Goldshire on the RP Realms (Whats the horde equivalent?)
- The fact that the alliance has resorted to trying to cheat/gain unfair advantage in WG on my server because they can't get enough people to join (Horde holds WG at least 95% of the time. Frankly, most of us would like a challenge in WG, the only time we get it is Tuesday evenings.
I've gone on too ling, could come up with plenty more =D

What annoys me is people who ask for an outrageous gearscore to run raids because they are too timid or impatient or whatever to teach people who have an appropriate
starting
GS.
Leaving gearscore out of the equation, because it's possible to evaluate gear without it, I find that a lot of under-geared players have no understanding that 2 different raid groups doing the same raid, with different objectives, can have different gear requirements and can be completely justified in asking for them.
Having correct starting gear for ICC means exactly that- you have the gear that you need to start learning it. When we all started doing ICC, we didn't kill the lich king the first or second week. We went in and wiped, week after week, on different bosses while we gained experience, and each week we earned the gear that we needed to get further and down more difficult bosses. Lord Marrowgar does not require the same kind of gear as the Lich King.
If you have enough gear to start ICC, it doesn't mean that you have the gear you would need to be able to down the Lich King, Sindy, etc. in a single raid period. And, if you don't have a lot of ICC gear, it means you haven't been in there very much, and won't know the later fights. Even if you have another toon that has more experience, there is a difference between knowing the fights, and knowing the fights with the class you're playing right now.
So, if your gear is not what the raid leaders who pug on your server require, it is completely reasonable that they migh actually know what they're talking about. If someone is pugging a raid, they usually only have a single raid period to finish it. So, if they want to kill the Lich King that week, it means taking people who don't need to be taught Rotface and Festergut, not wiping 3 or 4 times on Blood Queen because the heals don't have the gear to keep up, etc.
If you are just starting ICC, this late in the expansion, it means that you're pretty far behind most people who are pugging raids. Look for people doing alt runs for 4-6 bosses, or for a guild run where they are planning on learning together. Don't be bitter that people who have progressed further than you, who don't know you from Adam, won't give up their own progression that week to help you catch up to them.

I have no tolerance for people unwilling or too ignorant to improve.
Option 3 is like 100% chance for me. Have you tried whispering them instead of announcing their mistakes in party chat?

Yes, I know my gear isn't top percentage (of rattata) , but the only way I can get better is by getting gear from dungeons and learning them.
Oh, and know-it-alls who think knowledge+playing-since-vannila+grammar=better than everyone else.

A few things annoy me, I'm pretty laid back so something has to be really annoying for it to even make it on my "list":
-Elitists
-People who think negative reinforcement works...
-DPS that DPS the LAST mob when I'm LoSing a bunch of mobs to group up...
-"gogogo"
-"advice"... no one asks, yet everyone offers...
-People complaining about performance in heroics...
That only is justified if the complaints are illegitimate.
