Before the introduction of ToC and the addition of a harder version of the instance (Heroic mode as we now know it) the term "Heroic" merely refered to the 25 player difficulty of a raid instance.
Normal and Heroic raid instances pre-ToC are simply the 10 and 25 player difficulties similar to how they work today. Heroic isn't actually harder, it's simply tuned for 25 players opposed to 10.

25man was called "Heroic" and 10man normal. In Ulduar you could say Heroic were the "hardmodes".

There were also the triggered hard modes in Ulduar and OS that added mechanics but also dropped a few pieces of extra loot, as well as got you a few extra achievements.
