Walking into one of my first heroics with my freshly 70'd hunter (at the time, also my first toon)..
Had done a few others, but some over-geared raiding guildies graciously allowed me to come through Magisters with them. We get to Kael at the end and our paladin says:
Her: Okay, there's a new mechanic to this fight.. you need to MD him to the priest (they're RL friends)
Me: You sure? That seems... wrong somehow.
Her: No, it's cool. Just MD to the priest and the rest will work itself out.
Me: Oooookaaaaaaay....
So.. we get all set up, tank says "Okay kam, do your MD pull"...
They all see the crosshairs over the priest and I hear "What the..." on vent as I unload on Kael.
/wipe in spectacular fashion
Afterwards in vent:
Priest thunders: What the ^@$@ did you do that for?
I timidly reply: New mechanic?
Priest queries: <paladin>? You messing with the newbies again?
Paladin: <sounds of voice-activated rofl'ing>
We had a good laugh, but I earned my badge of Epic Failure that day.

I have the same problem as that Warrior, I cant inspect/promote/any other action which needs a rightclick on portrait or so. Its some bug many ppl have. (However he couldve used /inspect...)

I cant even remember what it was that dropped.. but about one time I was running MGT with two friends and a couple Pugger's (uber Fail right there...). I was on my lock, one of my friends was the tank, and the Group leader was one of the puggers.. (not sure how that happened). And some Epic tank thingy dropped, and I jokingly /roll.. but the thing is, I rolled 1-1000000, and got something like a 700,000+ roll, and the Lead gave me the item...
I spent the next 6 days with a GM ticket before I finally got the item to my friend.

Story of an epic Fail from my pal.
While looking for a tank for Shattered Halls back in TBC my freind (who was a pally healer) was getting a tad desperate.
So finding a i think warr ,who was all tank geared and speced quite well.(i think, my memory is a little foggy) they then zone in. Now keep in mind this was normal so there is a low chance that ppl can wipe (unless they dont know what there doing) they zone in and buff up.
Now the first few trash went ok (with a him almost over aggroing with a few heals) they get to the part were its 2 pulls in one. So they pull the first grp and almost immedetly my pal gets aggro and they wipe badly.
My Friend: Tank?
Tank: Yea?
My Friend: How are u trying to hold aggro?
Tank: Whats Agrro?
Lets say lots of face palming and they dispanded shortly after that.
