I've been using
5/11/55. Unholy Command isn't worth those two points and Virulence is good for PvP, but those points can be more valuable elsewhere. Summon Gargoyle is a good dps skill, but your RP should be going to Rune Strikes and Death Coil whenever available.
Wandering Plague might sometimes strike targets you don't want struck and are not being CC'd. (The Worshippers from Grand Widow Faerlina come to mind)
Blood-Caked blade is just a little bit of threat increase. I put those points there out of habit, but Two-Handed Weapon Specialization or Bladed-armor might be just as effective.
