I only gank if I've been subject to it repeatedly on that server. Also, when questing I only attack other players if they attack me, steal a kill, or attack a party member.
Most of the time I mess with them, sap and leave on my rogue... chains of ice on my DK. I don't gank the players, but I camp town npcs occasionally. Refuge Pointe is a fun one on my Horde server, as is Splintertree Post on my Alliance server.

I play on a PvE server because I don't like being disturbed by constant ganking while out questing, but I did have a very fun time once taking out Auberdine in Darkshore single-handedly. As a mage, it was relatively easy to run in and spam funny emotes on the lowbies while half a dozen Alliance night-elf guards twenty levels or more below me were flailing away at me without landing a hit. I slaughtered the guards for about fifteen minutes before an 80 DK came by and three-shotted me.
Was very fun, though, to see all the Alliance newbies milling about in confusion at the skull-level horde mage sitting in the middle of the alliance town and /cheering them while being attacked by guards.

I don't attack unless provoked, or typically if the opposing faction is killing quest-givers or any other NPCs that serve a useful function.
I had that mind set today. Was questing in the same area as a NE hunter and I could have ganked him since he hasn't seen me yet or left him alone so he could quest. So I do the later and 2 minutes later he attacks me while I'm fighting 3 mobs lol. So I died. Then he died. Then I died. Then he died. Then I died. Then he partied with a DK so I ran away :P

Meh, I used to gank people and then meet up with them again in other places at a higher level. I usually level faster than others, so I gank them for the lol and show them how much I leveled. Other times, I just gank people I know I can take out when I leave an area. Usually I expect them to bring in someone else only to waste their time XD
But one time, a Priest walked over. I saw her multiple times, we understood each other and didn't gank. Then she used Mind Control on me, but instead buffed me :D

Depends on mood, and several other things.
I was hanging around Teldrassil after a failed Raid a while ago, and I ran into the starting area and followed a lvl3 NE druid around, waiting for him to tap stuff, then I'd kill it.
Earlier today, I was in Crystalsong Forest, saw a flagged 79 NE Hunter, was on my 79 Rogue. I ganked him 3 times, he spirit rezzed. Saw him like 3 minutes later, killed him again, then left him alone. 2 shotting things is fun =)
I've sat around Goldshire waiting for lowbies to come by, flagged, and I'd sap them, wait for it to run out, garrote, rupture, vanish and just watch as they run around as their health drops. Quickly.
I don't have any specific rules for ganking. Except unless their named Legolass or Xxarthas or something, then they HAVE to die.
