I've wondered some myself. I know some people in RL that were having troubles with dps and it all boiled down to spec and rotation. Granted, it was the first 80 for both of them, and they weren't trying to get in raids and do heroics without my help or that of other mutual friends. Once I got them both pointed in the right direction(go to

and READ!) they gained about 1000 dps each.

I hate to be used to boost other DPS. I mean, as a furywarrior my DPS in heroics especiely on trash is second to none(3k+ mostly) but ffs how it sucks to have a 4/5 t7 geared feral druid with a PvE spec doing 1200DPS...
In my last heroic COT timed run the tank was second on DPS...We made it with 2minutes to spare, both DPS was -1500...

Anyways, you are quite correct on general failure DPS.
DPS is simply getting hit capped, and expertise capped if you class requires it, then performing the proper rotation. Too many noobs complain about being in bad gear, or are using improper gear. The only 'real' gear a dps class needs is both those caps, then they can preform their rotation properly, from there its simply grabbing epics for greater numbers.
Despite the simplicity, many players have basic issues with exercising this. Mostly it is because WoW is an easy game to level in, but once a player hits the level cap the game becomes significantly more complex.

I feel casters can also see a large gain in their damage output if they use a cast bar with a latency marker, particularly if they suffer from high latency. While this would obviously be true in cases with 300ms or more, even running at 50ms to 150ms (depending on the server and time), I can easily begin a new cast .3 seconds earlier than if I had waited for the spell to finish on my screen.
Unfortunately, most people don't recognize this issue because they feel mods cheapen the experience.

OP: The answer to your question is simple.
Laziness.
People come into heroics and/or raids just facerolling their soloing rotation, because hey, it works for dailies, right? Not thinking that there are wholly different mechanics for these encounters ("dps race", anyone?), they just tap 1, 2, 3, 4 with little to no modicum of urgency or attention.
As a prot paladin wearing Naxx gear, I pull, on average, probably 1500 dps when i'm tanking. Maybe a hair more if I'm going full-out with Exorcism, Consecrate on every cooldown, etc. Proabably... oh, 80% of the time I'm tanking a heroic I'm #2 or #3 on the dps list. If I see myself at #1 on the dps charts on anything but a chain-pulled undead pack, I leave group, because it's just going to be painful.
So yes, the answer is simply: laziness.
