1) Restoration and Elemental have very similar gear, except at 80, elemental needs spell hit, whereas restoration doesn't. Restoration will also prize mp5 higher, where elemental prizes haste and crit higher (though all 3 are useful to both, just different priorities).
2) Go for leather or cloth if they're upgrades. The only issue is that they tend to carry spirit which is worthless to any shaman. Even with the spirit, if they're an upgrade, snag them, just pretend it says +0 spirit.
3) Weapon is like armor, you go with what gives you the best total stats (Staff vs 1h + offhand or shield). Once you get into Outland, you'll see the shields and 1h weapons are itemized well for shamans. The bonus you get is that the shields can get their own int enchant.

It's not that it's less mana-efficient, it's just that more mp5 means more effective mana, means more healing should the fight drag out or hit huge damage spikes throughout.

Is this purely on account of restos not having Unrelenting Storm? Or is healing in general just less mana-efficient?
Healers are throwing out more mana per second than elemental shamans are. We have to have as much mana regen as possible. Yes, resto would love to have something like Unrelenting Storm, then there would be nearly no difference in the resto/ele gear (other than +hit).
As far as your HW/LHW macro question, it sounds to me like you need to get yourself Healbot and not worry about macros like that. Healbot puts a bar for each member of your party/raid telling you how many hit points they've lost, and you just assign clicks for clicking on their name.
Here's how mine is set up.
Left click - Chain Heal
Shift Left Click - Riptide
Right Click - Healing Wave
Shift Right Click - Lesser Healing Wave
Alt Left Click - Cleanse
