I was doing dailies in icecrown for 3 hours wondering why I wasn't geting my drop.

It wasn't me, but in a 25 man Naxx a hunter had on aspect of the daze on during the Heigan fight. When it came to the dance, half the raid died.
I was questing on my shaman in Westfall and I kept dying. I finally looked at my talents when I remembered it was after a big patch.

I'm a feral kitty druid, and during an instance I had to rez someone who had died. I have my rez spell on the 2nd action bar on my normal form, and when I switched back to kitty form and tried to DPS the next mob, none of my abilities were working. I was so confused and started panicking because I couldn't attack anything.
It wasn't until 3 mobs later I realized I hadn't switched back to my 1st action bar with all my cat abilities... *doh*

Ran Reg TOC. Tanking on DK I was taking some insane damage AND couldn't figure out why but i was pulling 3k dps.... Yeah I was in DPS spec and blood pressence.
OS 10 Burn couldn't figure out why I was pulling 3k dps. I was missing my Offhand.

Well, in Old Kingdom, I was marking the groups of 4 near the Bonegrinders. Just to save some time, I marked the Bonegrinder for after we finish killing the groups. So, the tank runs into the first group and we start dpsing them down.
As soon as I hit my hunter's mark and pet attack macro, my pet dashes off in a completely different direction. I didn't hit "Passive" fast enough, and I had enough time to facepalm to myself before the Bonegrinder stepped over my pet's corpse and ate the group alive.

I had a 37 gnome mage on a server I didn't play anymore. An old guildie from that server popped in on my new server one night and asked if I felt like dusting of my mage and come play for a couple of hours, so I logged onto the mage that I hadn't played for maybe a year.
I was met by some old guildies that also had chars around lvl 37, so we made a group and went to an instance. About halfway into the instance I started wondering why I sucked so bad, only to discover that all my talent points had been resat in a patch. I told the others and we all had a laugh about it, and then I took a short break to spend some talent points. "Hmm, frost looks good" I thought and put all 28 points in frost. Two pulls later I still sucked, so I checked my gear and found out I was fully dressed in +fire damage gear, lol..

Running a whole daily hc only to find the leader hadnt set it to hc...
i did this 2 days ago. formed a group of 4 guildies and a pug...
...but don't blame it on the leader alone! how do FIVE people not notice there are no emblems of conquest dropping, that everything is dying in 5 seconds or less and that the last boss didn't drop an epic?
that's right... no one noticed until someone said they couldn't loot the daily item.... /facepalm
thankfully everyone was a good sport and we ran it again on heroic ^_^
