Starting in Battle Stance
/cast Charge
/cast Berserker Stance
/cast Slam
Works fine for me. You do realise you'll lose Rage Stance Dancing after the Charge though?

You will, of course, have to put the macro in BOTH your Battle Stance toolbar and your berserker stance toolbar. It may be more efficient to put ONLY
/cast charge
/cast Berserker Stance
in the battle stance bar, and put
/cast slam
in the same slot on your berserker bar. After all, you can't charge in 'zerker stance anyway, and recasting Berserker Stance would be redundant.

Better Option...
New Macro, give it a name and select the ? icon
#showtooltip
/cast Charge
/cast Berserker Stance
/cast Slam
What this does: If your in Battle Stance (stance:1), it will show Charge as your icon. Once you hit the button you will do Charge and Immediatly change to Berserker Stance (stance:3). The icon on your button will change to show Slam. Hit the button again and you will do your slam ability. To get it back to the beginning you only need to switch back to your Battle Stance.
Note: I dont have a warrior so dont know if Charge is on the GCD. If it is let me know and I'll fix the macro to something that works with it.

Except of course, when you switch stances it pulls up your Berserker Stance control bar...and even if charge is off the GCD, you can't use slam while moving (which charge does) because it has a casting time, so you'll need to have the macro in both places, or put only the first two lines in the macro and just put slam on your berserker bar in the same slot.
