Agro and Threat

ok, since starting WoW with Classic as DPS i have always been taught to watch your threat and always keep it below the Tanks. Even now when i am DPS i do this, and if i do over agro by mistake, i always have a cd i can pop to reduce my threat.
I now Tank (Feral Bear). and 95% of the time i can hold threat fine, with the odd mob breaking off and going for the DPS in the group. Tonight i was in HC Vortex Pinnacle. First group of trash. Everyone is ready, i go in pop Challenging Roar, Growl at any melee i miss and go to where the caster mob is and start to keep threat up. I cant. The DPS and Healer are doing over so much threat that I cant re-gain threat, and even when Challenging Road and Growl come of CD, then i still cant generate enough threat to re-group the mobs off them. Needless to say we nearly wipe with all but me dying. OFC me being the tank i get the blame.
The question is this... Am i completely at fault here for not being able to over agro the dps/healer to pick up the adds again, or is it down to the DPS/Healer not being aware of their threat generation?

I believe Growl has diminishing returns (if you keep using it on the same mob, it lessens the effect).
I would pool some rage, charge in and spam a few AOE abilities (swipe & thrash).

For that particular pull, I'd do a crazy weaving trick where I (facepull) charge into one of the casters, Skull Bash + Lacerate the other if possible to force it closer and get a good chunk of initial threat, then (feral) charge back to the original target and just start smashing away at anything close enough, using Skull Bash to force the casters closer and closer together. It's not perfect, but it just might work.
