In a heroic - Combat > Mutilate due to AoE damage
In 10/25mans, especially on bosses - Mutilate > Combat... but you really have to become a mean cooldown-watching machine in order to keep HfB up. But it's totally worth it.

HfB is so annoying. 1 month of watching 30 sec tick by then- hunger for blood has ruinined mutilate fun :( They really need to fix how its rebuffed -.-. Soon as i get good wepons im going combat ! Word of warning.

Preference is a huge part of this game, copying the cookie cutters is not the best idea in the world. I play Mutilate because it's the more challenging spec to master in my opinion, why I enjoy it. I had enough of Combat during BC and got really tired of it, Mut has given me new things to do and pay attention to. Sometimes it borders on overwhelming in a few fights, but I find fun in that.
Some of the simpliest changes in spec to tailor to how you play can pay off big while the general populace my look down on you for using it. Example: Vigor. I get flak from a few people for having Vigor+Glyph in my spec, thinking the 1% crit from CQC I'm missing out on is worth more. On Patchwerk, you'd be right. On any fight where I have to leave range for any reason whatsoever, they're wrong. And that's about 75% of the Naxx fights.
