Surveys tend to require an approval from one of the mods. Usually it's done by sending an e-mail to Malgayne asking for permission, but there have been cases where a mod will approve it after it being posted. I don't know how they decide if it's okay or not, but I think the ones that don't ask for personal information tend to get approved, where the ones that do tend to get a lock and a link deletion.
EDIT: Third bullet in the "Welcome to the Wowhead forums" post: "If you elect to post a survey, please do so in our Off-Topic forum. A staff member will have to approve your thread to make sure that your survey is legitimate, otherwise the thread will be locked and the link removed."
