.So, really, where do you actually practice and get better at these raiding skills other than getting into a raid? For that matter, a current-content raid, as doing old content which is out-geared dramatically is, well, unhelpful at best.
Where do you find a patient group that is willing to teach you, or learn with you?
So honestly, what do you do to get the skills to perform well in a raid, when outside of raid content doesn't support it?
You practice in 5-mans then you practice in raids. With the LFR you get you practice on a 25 man group. After that you look for a newly formed raid group and join them, letting them know you're new to tanking/healing and hope they let you stay. Or you try to form a raid of your own with others knowing what they are joining.
My raiding group needed a healer, so I leveled one. I told them it was my first one so not to expect anything great from it. They understood that and took me along. I steadily got better and better. Learned when to use my CDs so I don't run out of mana or when extra healing is needed. As for tanking, it is again just learned as you go along. Depending on the fight you'll use certain CDs at certain fights.
Reading up on fights also helps to get better. Websites such as Wowhead, Icy-Veins and various Youtube videos with the addition of the Dungeon Journal are all great things to look at. Along with DBM it will let you know when certain things are coming and when you should wait to use an ability.
Starting out a tank/healer is annoying, but once you get one done the others are quite easy. After leveling my Shaman healer, getting a Priest and Pally healer were easier. Once Mists comes out though a lot of things are being changed around, so even more practice will be needed.

Tanking: Welcome to wtf. Can you train in 5 mans? Totally. But, as soon as you go in with more gear than minimal, you're making your job easy, which is something that won't be happening in a raid. Outgear DS? Not unless you're running HMs. Outgear HoT Heroics? PFFT! Cake!
If you feel overgeared you can either:
pull more
tank bosses without any healing
drop your pants (or whichever part of your armor you look siliest without)

Discuss with other tanks for ideas (or watch some
movies
with
ideas
, or watch some movies from tanks pov.) Practice on your own time.
Do more content, especially while high fevered, drunk etc. There is always some more stuff to learn.
Work on your UI, so its more suited to your needs and lets you see everything in a glimpse of an eye.
Learn the content, by both reading up all the tactics, watching movies and trying new things yourself.
