I dont see any changes to the ghoul so just keep macroing it.
What else would you want to do but claw, btw?

When I'm out doing dailies sometimes I attempt to stun an npc and it takes me mashing my key to get it to come out even at the start of the fight. It could be that the ghoul for some reason is slightly bugging out too though.
I think that's because Gnaw costs a lot of energy and claw's on autocast to, so you have to wait 'til it has enough energy to stun.
