Make under 10 or maybe 20 levels non-flagged.
Increase city patrols and add stealth-seeing guard "dogs" ... and if too many patrols are killed, the city guard gets called out, and floods the area with angry NPCs ... a realistic response to an invasion attempt.
Allow friendly fire ( right click on target and choose make hostile ), but have it cost reputation, based on the rep of the victim ... and have it turn the "friendly" firer red for all factions, requiring a five minute cooldown after the last blow.
Allow attacks in Dal and Shatt ... but have the Naaru or the Dal mages paralyze and then teleport offenders out of town, and preferably over a large drop.

Normal-RP realms didn't used to exist in the wee days of the game.
Wrong, there was Normal-RP realms at release.
It's the PvP-RP realms that was launched at least one year after release
Leveling on one of these servers would be horrible, and not in a fun way. It'd be like Southshore/Tarren Mill....everywhere. Ugh.
A lot of corpseruns. Hm... rolling a NE (wisp if dead) rogue/druid (stealth) would be quite WIN! :P
not sure about this idea. Sure pvp is meant to be war but sanctuary is sanctuary. Would look funny seeing someone trying to make war in shattrah with a big arse naaru teleporting you to a rock in nether who takes 30 mins from nearest land.
or dalaran who is also a sanctuary, hell if you watched the ulduar video you know the mages do not tolerate hostile interaction bcs then garrosh and varian tried to fight rhonin simple blasted them apart with a arcane explosion.
Also i know cities are meant to be slaughter fest but i think they should instead make a sanctuary level. Like you are under lvl 25 the horde cannot attack you due to bank alts and level players but then hordes can really take revenge on the more higher level allies. Would end the ganking lowbie debate there. ofc the said player under that lvl cannot also attack if not turn a pvp options on which makes horde able to kill you.
all leveling starter areas should stay sanctuary, the conquest vs sanctuary areas works well as it is really. Seriosuly who would want to pvp if 80 hordes waited in the lvl 1-10 zone and started killing new players, that is jsut tragic. PvP is meant as combat not nub killing omg i am so powerful i just killed a target who is like 70 levels under me lol.
An idea i did like is eve, where you could have cities as a guild. GEtting money etc, something i think would fit good in the maelstrom expansion where lets say there was 10-15 islands with towns you could make war and own. Also should be options to hire npc soldiers and weapons to make it not a push over and a certain time you can attack the town, lets say an hour which can vary btw 09-21 which the guild who takes the island can decide once or just make it a certain time on the day automatically. So the guild can defend the town somehow and not be offline and someone just grab and steal it.
A better idea would be an 80-only server with these rules.
