I'm pretty sure it is 100% per disease if you do not include the Death Strike Glyph (which I'm not sure if it actually works or is still bugged). I don't think the Ebon Fever or whatever it's called counts either.

You press the button, it hits, does damage, and heals you.
I believe
Ebon Plague
is considered a disease and therefore buffs it along with all your other disease-based abilities. I could be wrong, but I'm fairly certain I'm not.

Deathstrike heal for 100% of damage per disease but ebon plague count as an half-disease.

I tested. 1 disease = 100%, 2 disease = 200%, 2 dizz + ep = 250%.
I didnt check only ebon (which can only happen if someone dispell your disease) or 1 dizz + ebon.
