Firstly, it's Icy Touch that puts Frost Fever up.
Secondly, a lot of builds either have
Glacier Rot
or
Rage of Rivendare
, both of which grant extra damage to targets with Blood Plauge.
Ofcourse, it doesn't really matter for a Blood build without Glaicer Rot.

Its all about specc.. As Deepthought said if you have Glacier Rot its better to start with Plauge Strike and the Icy Touch.. If not I say use Icy first

Icy Touch while running up, Plague Strike when in range.
Both spells are low damage (the dots are where the damage is), so plague strike glyph seems so useless (maybe glyph + 3.0.8 talents, it would make it better for damage) but right now, it's just used to put blood plague on the target.
