If this has only been occurring since 4.0.6 or 4.0.6a then it's probably related to the patch, although I haven't heard of anyone else having the same issue yet. Otherwise, it might be an issue with your internet connection. Unfortunately, I can't offer any advice past rebooting your modem, router, and pc.
Sometimes this problem can be caused by a corrupted local cache. Go into your wow directory, into the "Cache" folder and delete everything. That will force the game to reload the data, and will hopefully fix your problem.
I've actually been having a problem with looting, and I don't know if it's related to OP's issue or not. Sometimes, maybe one in every 30 mobs, I won't be able to loot it. It's kind of frustrating. It started when 4.0.6 hit, and I haven't tried deleting my cache folder yet. Will report back once I get home.
If you can't loot because the cursor's grey, that's probably the old bug where the corpse is not where your client thinks it is. This used to be way worse back in the 1.x days but I've seen it a few times this week as well. Usually the only way to get that loot is a quick relog, since then your client gets the correct corpse location.
I've mined a few mineral nodes in Deepholm recently where I got the XP but no loot. The loot window didn't open and the node disappeared. I put it down to phasing.
Also killed a mob just as it got punted by an NPC. I went to go loot it but I was told "You are too far away". The client probably thought my loot was located where the NPC punted.
