Both Oblit and HS require diseases for boosted damage. I think it works with another DKs diseases (but probably not, haven't tested it), but if you're not putting your own on you'll be missing out on a lot of dps from the DoTs.

It really depends on what you're doing.
If you plan on tanking I'd definitley go for epidemic, but that's a personal choice just because I really like the extra time on diseases. Mostly I leveled that way, and it's just easier moving into taking with disease times I'm used to working with for rotations. This is my spec (Blood Tank) and it works for me pretty well atm. (I'm doing herocis, I haven't jumped into raids yet)

For DPS too I'd say it's a good choice, it allows you to save a round of runes for damage rather then re-applying diseases.
Again it all really comes down to what you feel comfortable working with and what feels like a good flow for you. If you find yourself hoping for another DK to help with diseases, then I'd grab Epidemic. If not, that's up to what you feel good with.

If you're using Blood for PvE DPS, it should looks something like this.

Epidemic gives you much more DPS by saving Frost and Unholy Runes for Obliterate or Heart Strike if they are converted into Death Runes. Annilhilation is a must if you want to use Obliterate. You'll be wasting Frost and Unholy Runes putting up your diseases if you don't have it.
