For an example, look at the drops for
Red Winter Hat
The first few are hat-bearers of this year and others, while the other drops are by random mobs. One single drop off each humanoid.

My brother got a drop in Thousand Needles at 29. Needless to say I'm peeved at 52.

Strange because, every single time i have been to 'Heroic : Ahn'kahet: The Old Kingdom' Jedoga Shadowseeker has always dropped one...same with Grand Magus Telestra in 'Heroic: Nexus'

Certain NPCs always drops them; generaly Humanoid bosses in Wrath Dungeons (Normal and heroic.)

The droprate is about the same as getting the pumpkin-bag during hallow's end.
In other words: It's pure chance.
