There are two primary possibilities to check.
First: Are you exiting the game properly? If you are not using the /exit command, or the Exit Game button in the game menu, or otherwise accessing that command, your settings may not save properly. For instance, if you exit the game by using Alt+F4.
Second: Does the game have permission to write the settings files? This should usually be the case, but sometimes (generally on Windows Vista), the game is run by a user with insufficient permission to create the settings files in the appropriate locations. Unfortunately, I'm using Windows XP, and I'm not sure how you might go about fixing this problem. The Tech Support forum on the official site should be a good resource.

I've had this probelm before, especially when copying WTF folders and the like. Assuming you're using Windows, right click on your "World of Warcraft" folder and make sure that the "Read Only" box isn't checked. When you apply, make sure you're applying to all subdirectories as well. This has solved the problem for me in the past.
