Leveling spell rotation:
VT -> MB -> SW:P -> MF -> PW:S -> MF -> MB and alternative after that one -> SWD
Mob dead, and I got both Spirit Tap and Replenish ticking, which gives back ALOT of my mana.
Raid/HC spellrotation:
SW: P -> VE (if fully specced) -> DP -> VT -> MB -> SW:D -> MF -> MF -> MB and repeat until the mob is dead.

You shouldnt be doing SW:P so early in the rotation. Since mind flay refreshes SW:P on your target, you want to have all 5 stacks of shadow weaving before you do it. VT->DP->MB->SW:D->SW:P->MF seems to be the rotation that I get highest dps on a single target. And of course keep all dots refreshed and MB/SW:D always on CD. For multi-target I'll do the same rotation on the initial mob to get the shadow weaving stacks, then I tab-dot VT and SW:P on the rest, followed up by mind sear. As soon as the initial target drops I go about my regular rotation on the next focused mob. Since the tank I always do things with is a paladin, threat is not a problem, so I'm not sure how this will work out with other tanks. Hope this is useful.
