Buttons 4 and 5 on the mouse (left-side buttons) will work as-is. They're valid mouse buttons.
Anything past 5, bind to obscure keyboard keys. Eg, buttons 6-9 on my mouse are bound to F5-F8. Then when you go in-game, the game will recognize these buttons as normal keyboard keys, and can bind them.
By the way, you're thinking of your bindings totally the wrong way, especially for PvP. First rule of good binding is: place vital things close to your index finger and work progressively outward from there; 2nd rule is: keep things logically categorized. (
Edit:
Third rule is, never step backwards or turn with the keyboard. Turn with your mouse, and use only strafe/forward on the keyboard!)
After several years of PvP, I honestly can't believe anyone still uses 1-N keys as their main binds. If you want some ideas of how I do it, take a look at my binding setup for some classes.

My odd mouse buttons are using , . .
As far as actually using a mouse button as one of the keyboard modifier keys goes, that's definitely up to the way the mouse software works. Though, if anyone would make software that would do it properly, I'd hope razor would. Perhaps there's a different sort of binding you need to do for the modifier keys? Look for a direct binding rather than a binding to a macro?

Yeah, I'm using one of their headsets because it's the only one I found that had an around-the-ear cup and a mic boom. I need the former because I wear glasses and 4 hours of raiding with my ears being pushed into them makes me sad.
They died in less than a year; but, I got an RMA with pretty much no difficulty and got a new set in about a week. These are okay so far.
But, yeah. I have some flavor of the opinion I have about Monster for Razer.

It seems mouse and keyboard software is always ineffective. I rely on the in-game control for more robust control of my toon. As a rogue, you have a metric crap-ton of skills many of which are dependent upon position and stance.
Using a 5-button mouse with software disabled so that all buttons simply act as buttons (sometimes just getting this should be Feat of Strength) I use the following macro's;
Left Button
#showtooltip
/cast Ambush
/cast sinister strike
/cast Garrote
/cast gouge
/cast Pick Pocket
/cast Rupture
/startattack
Middle Button
#showtooltip
/castsequence Fan of Knives
/cast Blade Flurry
/castsequence reset=23 shadowstep,Kidney Shot
/use Mogu Rune of Paralysis
Right Button

It took some time to get the muscle memory to match the bindings and somethings had to go in some apparently illogical places simply because I was so used to them being there, but it works fine. From a 5-button mouse you get 18 control commands dependant upon stealth / nostealth and nomod / shift / ctrl modifiers. I never have to take my hand off the mouse to maneuver and there is still room for 1,2,3,4 to be easy, quickly available macro's as well, also context-sensitive to stealth and modifier toggles.
